FEES

A fee schedule is attached, and payment (or co-pay) is due at time of service.  We offer VISA, MasterCard and Discover as an alternative to cash payment.  There will be a $10.00 processing charge for any check returned to us uncollected by the bank.  Any unpaid balance that is not received by The Colorado Family Center, P.C. within 60 days after the date of billing will accrue a monthly late charge of 1.5% of the unpaid balance. 

INSURANCE

While court-related work is not covered by insurance, many health insurance policies cover psychological services.  Please check with your insurance agent to learn the extent of your coverage.   Each staff member is a participating provider in various EAP, PPO and managed care plans.  Our office can assist you in determining whether a provider is affiliated with your plan. 

As an added service to you, we will file your claim so you will receive your reimbursement quickly.  We do not, however, negotiate settlements on a disputed claim.  You will receive a monthly statement that includes the information sent to your insurance.  If your company needs more information, we will be glad to provide it.
